Job Summary:
The Community Health Navigator is responsible for ensuring individuals receive appropriate intervention and follow-up of needed community services and support in the COVID-19 Pandemic.

Essential Functions:

• Prepare and write a wide range of reports, such as recording clients' progress, charting referrals and scheduling.
• Ensure collection of program data.
• Report to the Program Manager and/or Director of Behavioral Health on a regular basis regarding overall program performance and progress.
• Ensure program/participant confidentiality.

Programmatic Duties
• Utilize a culturally-based approach to engaging American Indian (AI) individuals experiencing critical behavioral health.
• Conduct telehealth services.
• Track client information and provide proper follow-up in regards to referrals and appointments.
• Perform initial screenings to identify needs individuals experiencing critical behavioral health needs.
• Educate to increase knowledge of the availability of wellness and other resources for meeting financial needs associated with mental health treatment.
• Refer individuals to care providers and community resources.
• Serve as a community behavioral health resources.
• Establish and/or maintain professional relationships with community partners.
• Identify gaps in health and human services for AI youth and young adults at risk for suicide.
• Submit accurate written reports on participant progress.
• Participate in training sessions, meetings and conferences related to the Program.
• Maintain accurate records of activities, communications, and travel.
• Follow established GPTCHB organizational policies and procedures.
• Serve as an advocate for clients. Consistently demonstrate the ability to link client/families with community resources. Stay informed of relevant community resources in order to make appropriate referrals.
• Assist in conducting home visits and transporting clients when safe and possible.

Work Environment/Physical Demands
The characteristic demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.
While performing the duties of this job, the employee is frequently required to stand, walk, sit and use hands to finger, handle or feel. The employee is often required to stoop, kneel, crouch or crawl. The employee must regularly lift and/or move up to 25 pounds and frequently lift and/or move up to 50 pounds. The employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.

Travel: Frequent local travel required; occasional out-of-town travel required.

1. AA/AS in Social Work or related field plus one (1) years of direct service; or a minimum of (3) years in direct service experience.
2. Strong written and verbal communications skills. Must be able to present to, and interact with both public and private health officials at tribal and community levels.
3. Experience that indicates the ability to interact effectively with leaders among Indian communities, government agencies, scientific and academic communities, medical and health-related organizations, non-governmental groups and the public at large is required.
