Job Summary:                                                                                                                                                                                 

The Community Health Worker is responsible for overall implementation and accountability of the Great Plains Healthy Start (GPHS) program site located in Sisseton Wahpeton. 


 Essential Functions:                                                                                                                                                                       


  • Provide daily management of the GPHS project in accordance with all contractual and cooperative agreement, as well as funding agency
  • Ensure project objectives and deliverables are met and are in compliance with all GPTLHB and GPHS policies and procedures.
  • Assist with ensuring accurate and timely financial transactions that are consistent with GPTLHB

  • Report to the GPHS Program Manager and/or Director of Maternal and Child Health on a regular basis regarding overall program performance and
  • Attend and assist in all required classes, meetings, GPHS in-service training and required community

Programmatic Duties

  • Create, promote and disseminate information to community members to recruit program participants and raise public awareness and education regarding MCH
  • Aid in the development of appropriate service plans for enrolled
  • Conduct home visitations, provide support to mothers (and their partners) during pregnancy and until infant is 18 months of
  • Track client information and provide proper follow-up regarding referrals and
  • Update and coordinate the tracking of infant immunization and physician visit
  • Serve as an advocate for clients. Consistently demonstrate the ability to link client/families with community resources. Stay informed of relevant community resources to make appropriate referrals. Be proactive in participating with community resource programs partnerships.
  • Assist the Public Health Nurse in conducting home visits and transporting clients when possible.
  • Advocate for, and participate in, local inter-disciplinary Infant Mortality Review
  • Recruit local community action network (CAN) members, conduct CAN meetings, provide attendance records and minutes of said meetings. Participate in CAN

  1. Minimum education required is a high school diploma or GED with two to five years of administrative and/or case management

COVID-19 Vaccination/Immunizations

Upon hire and prior to patient contact, all new and existing employees will be required to provide documentation of current immunization status to include but not limited to: COVID-19, Influenza, Hepatitis B, MMR, Varicella, TDAP, and assessment of TB status. Immunization status will be reviewed per Privacy Act and provisions thereof. It is recommended that employees keep a copy of immunization records. The employee health RN prior to the new employee's orientation to department. If the employee is unable to obtain documentation of immunity, Employee Health RN will order labs and provide required vaccinations per OHC immunization requirements policy. Immunization records will be kept in the employee health file and individual patient record. Records will be secured in the office of the Employee Health Nurse. Records are governed by the
